Macy's Rebate Forms

Rebates must be submitted during the offer window and within 30 days of purchase. Please allow 6-8 weeks for delivery.

How to submit a rebate form:

  • Print out and complete the mail-in rebate form for the item(s) you purchased below
  • Attach your original sales receipt or packing slip and UPC label from the qualifying product.
    • UPC label is usually a 12-digit number located on the packaging below the barcode.
  • Review and fill out the required information on your rebate form.
  • Mail to the address shown on the form.
