Macy's Rebate Forms

Rebates must be submitted during the offer window and within 30 days of purchase. Please allow 6–8 weeks for rebate delivery after submission.

 

Follow these steps to submit your rebate:

  1. Enter online or attach (if mailing) your original sales receipt or packing slip and the UPC label from the qualifying product.
    • The UPC label is typically a 12‑digit number located on the product packaging below the barcode.
  2. Review and fill out all required information on the rebate form to ensure accuracy.
  3. Submit form online or mail your completed rebate form and required materials to the address listed on the form.
