Macy's Rebate Forms

Rebates must be submitted during the offer window and within 30 days of purchase. Please allow 6–8 weeks for rebate delivery after submission.

 

Follow these steps to submit your rebate:

 

  1. Print and complete the mail‑in rebate form for the qualifying item(s) you purchased.
  2. Attach your original sales receipt or packing slip and the UPC label from the qualifying product.
    • The UPC label is typically a 12‑digit number located on the product packaging below the barcode.
  3. Review and fill out all required information on the rebate form to ensure accuracy.
  4. Mail your completed rebate form and required materials to the address listed on the form.
